When we confirm your registration, you will receive a packet of forms and information, including a Health History and Examination form. These forms contain important information associated with the camp experience, so please read and complete the forms carefully. All forms must be completed and signed by participant and parent/s, as appropriate. Final acceptance to the camp session is contingent on our receipt and review of all forms.
