Terms & Conditions
o Please sign and return with your child's complete application: (Please keep one copy for your personal records)
o If full tuition is not made at this time or if paying by credit card, I agree to give a minimum $500/child deposit with this application plus a valid payment plan option in order to pay the tuition in full by May 1st, 2014. Failure to do so will immediately revoke the Early Bird Rate and I will be charged the full non-discounted tuition amount accordingly.
o The $500 deposit per child is non-refundable.
o Camp will offer a $25/week sibling discount if applicable. Full tuition for the first child and then the discount will be applied to each subsequent child.
o Camp Laurelwood has the right to deny admission to a camper for any reason. If your camper is denied admission, you will not be charged a deposit.
o Balance refunds will NOT be given after April 1st, 2014 unless a doctor's note is presented.
o I give permission for my child to participate in all Overnight Camp Activities (See website for full program offerings and details).
o I understand that Camp Laurelwood is not responsible for any loss or any damage of personal property and items sent to camp are at my OWN RISK.
o I give permission for child's picture to be used for Camp Laurelwood's publicity.
o I give permission for my child to go on Camp Laurelwood planned and supervised field trips. Campers will be transported by school bus and/or Camp authorized vehicles. Examples of field trips include ice skating, bowling, movies, amusement parks, bike trips, beach etc.
o I give permission for any remaining funds in my child (ren)'s Canteen account after camp be donated to the Camp Laurelwood Scholarship Fund.
o I understand that Camp Laurelwood is required to follow American Camp Association & State of Connecticut Health Department guidelines. If my Child's medical records are not completely updated per the Camp Nurse’s approval, my camper will not be allowed to enter camp until they are complete.
o I hereby give Camp Laurelwood permission to take my child to any hospital facility or outside doctor when deemed appropriate and I give permission to such hospital or outside doctor to authorize x-rays and emergency treatment if necessary. I understand services rendered by anyone other than camp's medical staff are my financial responsibility.
o I understand that Camp Laurelwood is no defined as an entity subject to HIPAA regulations concerning records. Certain situations may necessitate that my child's medical information be shared with the administrative staff, infirmary, and any camp staff who are deemed necessary. I give permission to any health care provider to share my child's medical information with the Camp doctors/nurses and other Camp medical staff for treatment.
In Connecticut, licensed Camps administering medications to children shall comply with all requirements as described in the CT State Statutes and Regulations.
Physical exams are valid for two years from date of last examination.
